Default Countries/States, Cases/deaths- Swine Flu

I'm already starting to lose track of which countries have officially confirmed (versus suspected) local cases of this new strain of Swine Flu.

Maybe we can build a current list and update it as new confirmations are added.

So far there have been official confirmations in these countries:

Mexico................... 18 confirmed cases?
US......................... 20 confirmed cases?
Canada.................. 4 confirmed cases?

... and others?


US States with Confirmed Cases:

California................. 7 cases
Kansas.................... 2 cases
New York................. 8 cases
Ohio........................ 1 case
Texas...................... 2 cases
TOTAL COUNT.......... 20 cases


We can add more detail for other countries as wanted.

So far the tests are positive for influenza A:

Probably via PCR (polymarese chain reaction) test-you can run a PCR to test for the seasonal H1N1-if that comes up negative (and other tests say it's H1N1-just not which subtype) then it most likely is the Swine Flu variant... I'm a little surprised NZ hasn't done that yet (maybe they don't have those facilities?)

Probably unrelated but ProMed noted a case in Spain in Nov 08. I thought it might be worth keeping on the radar as the source of the outbreak is investigated.

I read that New Zealand had to send the samples to Australia for testing, that is prob. the reason for the delay.

have they already put out a swine flu PCR reference sample?

How would that accomodate mutations as it moves around?

Some areas of the viral genome must be stable in order for the virus to be viable. Primers are generally designed around those areas that are the most stable.

And genetic drift takes on average ~18months (don't know the range) before the changes require a vaccine change.

With H5N1, it isn't circulating yet so a vaccine choice would be premature. One cannot predict the makeup of the strain that might circulate in the future. By necessity, it would have to be different just to spread efficiently H2H.

This new strain of H1N1 is already circulating. It won't be long before some predictions can be made about the likelihood it will still be circulating when the next flu vaccine production begins. That is only a month or so from now for the Northern hemisphere vaccine. It makes sense to add it to the mix.

Interesting in that there was already talk of adding a 4th strain but the recommendation being determined was going to be to add a 2nd B strain. Now we may get 3 A strains and 1 B strain instead.

The NZ cases are unconfirmed. At the moment they are Influenza A. There are also 3 unconfirmed possible cases in Australia.

Twitter reports that two cases in Scotland have been confirmed by the country's Health Minister. Sky News is reporting that at least 7 peopole who had come into contact with these two confirmed cases have developed similar symptoms. Toss Scotland on the list....

Also, add 20 new confirmed cases of swine flu in New York City, all students form the same school.

Since WHO seems slow to report, it's hard to know for sure which to consider "official" confirmations, so for now let's leave notes in cases we've only seen limited reference to. Plus maybe we can post notices of "official" government confirmations as we see them.

In Scotland if we have cases that did not travel to Mexico, is this a first occurance of transmission from someone not traveling to Mexico? If so I think it might be a big mental boundry with WHO that has just been crossed.

I don't think the first cases in CA or Texas had recently travelled to Mexico, had they?

But I think they would be the first cases that far away with no recent Mexico travel history.

Of the confirmed 2 in Kansas, only the husband had traveled to Mexico and the wife came down with it shortly after he returned home.

Two people admitted to a Scottish hospital after returning from Mexico have been confirmed as the first British cases of swine flu.

Scottish Health Secretary Nicola Sturgeon said both patients were recovering well.

Can someone cofirm NC/SC (USA)unconfirmed swine flu, I just had a patient come in and say they heard on news that NC and SC had swine flu. Much thanks will check thread between patients.

SC - Prep school students that had just returned from Mexico.
Also - same paper is reporting that in NC the Health Department, along with the director of Epidemeology(?) is saying there's some cases in NC that are being looked at as potential swine flu cases. Prelim tests results due weds - confirmation expected thursday

This may not apply in this case, but IIRC under the new international health regulations countries are supposed to report cases to WHO, but they are NOT obligated to allow their status to be made public.

Has anyone else heard of another country with confirmed cases today?

Default Does Confirmation matter?

Seems things are getting to the point where confirmation doesn't matter, and may be misleading. Out of all the cases being mentioned as possible, probable, or suspect, such as hundreds of students in New York, how many will actually be tested and then "confirmed." The idea that Mexico has less "confirmed" cases than the U.S. shows that "confirmed" focuses on testing. Testing which can't keep up with the growing numbers.

It may be time to add to reports on "confirmed" by trying to track how many people are infected according to reliable reports.

Totally agreed that the numbers of confirmed in any given area become meaningless after the virus is deemed endemic in that area and/or testing capabilities are overwhelmed.

For example, NYC has apparently just stopped testing anything other than serious cases, since they need resources elsewhere rather than confirming every mildly ill child in St. Francis Prep, for example.

It's still worth tracking areas reporting new cases, and for now we may as well track confirmations at least until they announce testing suspensions in given areas.

Maybe on our list we can start making notes of places we know have started limiting testing (eg NYC) or are not keeping up (Mexico).
